SCHOOL STAFF SCHEDULE
Stars Academy, LLC staff attempt to be readily available to any student that wishes to
speak with them, however sometimes schedules do not provide for an immediate meeting.
Because of this, students should arrange a meeting with any administrator through their instructor
or via telephone.
ATTENDANCE
The student is expected to attend all regular scheduled classes. In cases where absence is
unavoidable the student should make arrangements to cover the session missed in another
manner (such as class notes or homework). If more than one day is missed you will be dropped
from the class. We will need a written letter as to why you needed to miss class, at that point the review board will determine if your class tuition will be transferred to the next scheduled course. Late arrivals: If you are more than 15 minutes late to class you will receive a tardy, three will result in being dropped from the course.
STARS ACADEMY, LLC MISSION STATEMENT
Stars Academy meets the educational needs of students, employers and the community by designing a quality training program utilizing relevant phlebotomy practices.
PHILOSOPHY AND OBJECTIVES
The educational goal of Stars Academy, LLC is to provide every student with a basic working knowledge of the opportunities available in the area of training they have selected as a career field. To accomplish this, Stars Academy instructional staff members assist and instruct students to: understand the requirements for working in the health care industry, be aware of social skills, positive attitudes, quality interpersonal relations needed to succeed, develop awareness of continued training. Completion of the course is an agreement between the student and Stars Academy, LLC.
FACILITY
Stars Academy, LLC is located at 8077 West 95th Street, Hickory Hills, IL 60457 - PH: 1-708-233-0590 / Fax: 1-708-233-5806. Our facility simulates a hospital room and laboratory that is set up for teaching. Our Location maintains an occupancy permit from the city of Hickory Hills and the Illinois State Fire Marshall’s office.
PHLEBOTOMY
There is a growing need for phlebotomist in the Chicago Land area. The Phlebotomy field is constantly evolving and the need for multifunctional employees is very strong. In response to this demand Stars Academy has designed a training program for an individual who can assist doctors and office managers in a number of ways.
ADMISSION REQUIREMENTS
If the student is under 18 years of age, the enrollment contract and other documents must be signed by a parent or guardian. They must have a good command of the English Language both written and verbal. You must have proof of a High School diploma or a GED to enroll in the course. High School students will have to get permission from their counselors.
SATISFACTORY PROGRESS
Students must complete 30 classroom hours, 30 hours Lab/CPR, pass the final examination, and have attained a 70% or above grade average for the Phlebotomy program.
ACADEMIC POLICY
Students are informed of their academic progress at the completion of each week for the prior week. Students have the right to access their files at any time during class hours. Students are graded on a percent system you must receive a 70% or above as a final grade to pass the class.
TUITION AND REGISTRATION
Fees charged by Stars Academy, LLC included the cost of instruction, supplies, and text book. Tuition must be paid in full $1,600.00 by graduation. If student’s financial obligation is not met by graduation, the certificate of completion will be withheld until tuition is paid in full.
FACILITY POLICY
Maintaining and preserving school facilities and equipment are the obligation of all students and staff members. Stars Academy expects students to treat the facilities and equipment with care. Students will be held responsible for any destruction to school property. Any student who deliberately damages the equipment or facility will be expelled from the school.
SMOKING POLICY
Smoking is not allowed in the school per the state of Illinois
DRUG AND ALCOHOL AWARENESS POLICY
Stars Academy, LLC has a zero tolerance for drugs and alcohol. Any student caught with alcohol or drugs in their position will be expelled from the school.
DRESS CODE
Scrubs, Closed toed shoes. A full-length lab coat will be provided by the school to use for labs.
STUDENT COMPLAINT POLICY
Should any student have a grievance (unresolved complaint) about status, grades, records, faculty, or other, the normal recourse is for the student to consult with their instructor first then the complaint should be submitted in writing to the clinical director of Stars Academy, LLC or the following address:

GRADUATION REQUIREMENTS
Students will be awarded a certificate and a letter of recommendation outlining your training and experience.
CLASS CONDUCT
Mature behavior and conduct with the highest business standards are expected of each student. The school reserves the right to dismiss or suspend a student whose conduct is regarded as unsatisfactory and who engages in such conduct as: equipment sabotage, cheating and dishonesty, failure to adhere to school rules and regulations, disruptive behavior, destruction or theft of school property. Activity which infringes on the right of others and/or possession or consumption of alcohol beverages or illegal drugs on any part of the school property will be in violation of these rules. We retain the right to drop any student that has violated one of these rules.

Syllabus
All training and teaching aids, materials and phlebotomy supplies used in clinical training.
Use of all phlebotomy equipment, instrumentation and specialized phlebotomy and laboratory materials.
A certificate in Phlebotomy and a letter of recommendation outlining your training and experience will be awarded to all students who have attained a 70% or above grade average and paid in full.
CPR training and certification/card which complies with the American Heart Association.
DISMISSAL
All students are expected to maintain a satisfactory level of academic achievement, conduct themselves as responsible adults and to attend all classes on time. The school reserves the right to dismiss any student who:
1.Fails to maintain satisfactory academic progress.
2.Exhibits conduct which is found by the administration to be detrimental to the individual, other students, or the school.
3.Fails to meet attendance requirements.
4.Fails to meet financial obligations to the school as agreed upon.

FEDERAL TRUTH IN LENDING DISCLOSURE
STUDENTS RIGHT TO CANCEL
The student has the right to cancel the initial enrollment agreement until midnight of the fifth business day after the student has been admitted; If the right to cancel is not given to any prospective student at the time the agreement is signed, then the student has the right to cancel the agreement at any time and receive a refund on all monies paid to date with 10 days of cancellation. Cancellation should be submitted to the authorized official of the school in writing.
REFUND POLICY
Any student applying for a program that is discontinued by the school shall receive a complete refund of all fees and/or tuition paid. It is the policy of Stars Academy to issue refunds of tuition and fees in a prompt manner. As a matter of courtesy, students should give written notification to Stars Academy, in person or by registered mail, of their intention to withdraw from a program. If no notification is received, and a student has had an unexplained absence of more than (2) consecutive days, Stars Academy shall consider the student to have withdrawn from the program. In all cases, the date of withdrawal shall be the last day of attendance.
Refunds shall be made within 30 days of the last day of the attendance if written notification has been provided to the institution by the student; otherwise, refunds shall be made within 30 days from the date the institution terminates the student or determines that the student has withdrawn. Determination that a student has withdrawn shall be made within 30 days of the last day of attendance. Stars Academy shall provide written acknowledgement of a student’s notification of withdrawal within fifteen (15) calendar days of the postmark date of the notification of withdrawal. In all instances, refunds shall be based on and computed from the last day of attendance.
REFUND SCHEDULE
1.If a student does not begin the course, and fees/ tuition of $300 have been collected, no refund of fees shall be made. It will be held for application and registration fees. But the student will not be responsible for the remaining balance.
2.When notice of cancellation is given after the student’s completion of the first day of class, but prior to the student’s completion of 25% of the course of instruction, the school will retain the registration fee.
3.When a student has completed in excess of 50% of the course of instruction Stars
Academy, LLC will retain the registration fee and the second payment of $650.
4.When a student has completed an excess of 75% of a course of instruction, Stars Academy, LLC will retain the registration fee and the entire tuition that has been paid thus far.

YOUR TETANUS INJECTION MUST BE UP TO DATE.
The following information might be helpful for Phlebotomy students.
High school students who have graduated less than 7 years ago should be up to date. You still have to bring in records.
If you are 18 and under you DO NOT have to have your tetanus shot updated.
